Kombination der Antimalariamittel Mepacrin und Chloroquin bei therapieresistentem kutanem Lupus erythematodes

Zusammenfassung

In der systemischen Behandlung des kutanen Lupus erythematodes (LE) sind Malariachemotherapeutika Mittel der ersten Wahl. Einige Patienten zeigen jedoch unter einer Monotherapie mit Chloroquin oder Hydroxychloroquin keine Besserung des Hautbefundes. Wir berichten über eine 30jährige Patientin mit einem außergewöhnlich therapieresistenten kutanen LE, der sich unter dem klinischen Mischbild eines LE tumidus und eines subakut-kutanen LE präsentierte. Behandlungsversuche mit Chloroquin, Hydroxychloroquin, Dapson sowie Azathioprin, jeweils kombiniert mit wechselnden Dosen Prednisolon, zeigten keine Wirkung. Im weiteren Verlauf kam es trotz Fortführung der systemischen Medikation zu einer Progression der Hautveränderungen. Erst die Kombination der Antimalariamittel Chloroquin und Mepacrin führte nach vier Monaten zur Abheilung der Hautveränderungen.

Abstract

Antimalarials represent the first line in treatment of cutaneous lupus erythematosus (LE). However, some patients show no improvement on monotherapy with chloroquine or hydroxychloroquine. A 30-year-old female patient had treatment-resistant cutaneous LE exhibiting features of both LE tumidus and subacute cutaneous LE. Previously, the patient had been unsuccessfully treated with chloroquine, hydroxychloroquine, dapsone, and azathioprine, each in combination with variable doses of prednisolone. However, the LE lesions increased during these therapeutic regimens. A combination of chloroquine and mepacrine therapy led to improvement and then total clearing after 4 months of treatment.
